
This page is designed to help Bluebell couples prepare their title form for use in the DVD. You can download a titles form here if you require. (right click, save target as) Download Thank you/Titles Form PDF document Download Thank you/Titles Form Word Document The title form is used for the credits/thank you's shown on your DVD. At the end of the video the credits will start with the words " Special thanks to " followed by the list you provide. Decide who you would like to thank for their help with your wedding and write their names down on a plain sheet of paper. You can credit as many people as you wish, but long lists will have to be provided on floppy disk. ______________________________ The Titles FormLook at your title form. The first 2 lines are for the Bride and Groom as you would like to see at the start of your DVD. So the Bride will write her maiden name at this point. Use capitals and write using a ball point pen. You can use first name/surname, or include your middle name.

SUSAN BROWN Groom ........ROBERT WHITE This information will be used at the start of the video and will appear as "The Wedding of Susan Brown & Robert White" on the completed DVD. ______________________________ Now fill out the rest of the form as you would like to see names appear at the end of your DVD most Bluebell couples choose this format Bride's Mother .... MARY BROWN Bride's Father .......JACK BROWN etc. Chief Bridesmaid .. N/A Bridesmaids .......... SARAH WHITE, JULIE GREEN (this is a wedding example where there is no chief bridesmaid.) If you require additional titles please write them on the reverse of the form. example PETER BAKER FOR MAKING OUR CAKE LISA STITCH FOR THE BRIDESMAIDS DRESSES etc.
Now go over the list you have made. Check that you have written their names as they like to be known and the spellings are correct. always use capitals and write using a ball point pen. Finally, ask a third party to check it over for spelling mistakes and clarity. That's it! The form will be collected when I visit you. |
